Job summary

Declare Wellness Center seeks an experienced Nurse Practitioner or Physician Assistant who wants to practice medicine differently. You’ll have the time and support to build meaningful relationships with patients, understand what’s happening with their health, and guide them toward measurable improvements in how they feel and function.

Specialized clinical training is provided. Prior functional medicine or BHRT experience is welcomed but not required.

Qualifications

• Experienced Nurse Practitioner or Physician Assistant.
• Willing to be trained in functional medicine and BHRT.

Responsibilities

• Conduct patient consultations, review-of-findings appointments, and follow-up visits.
• Manage BHRT and hormone optimization care.
