Declare Wellness Center seeks an experienced Nurse Practitioner or Physician Assistant who wants to practice medicine differently. You’ll have the time and support to build meaningful relationships with patients, understand what’s happening with their health, and guide them toward measurable improvements in how they feel and function.
Specialized clinical training is provided. Prior functional medicine or BHRT experience is welcomed but not required.
Tired of practicing medicine in a system that gives you 15 minutes with a patient?
If you became a provider because you genuinely wanted to help people improve their health, but increasingly find yourself frustrated by rushed appointments, insurance requirements, prior authorizations, and high-volume patient care, this may be the opportunity you’ve been looking for.
A four-day work week. No weekends. No hospital call. No insurance billing. No prior authorizations.
And you do not need to arrive as a functional medicine expert.
Declare Wellness Center is an independent functional medicine practice founded by Dr. Craig Cohen, DC, FNP, and Amanda Cohen in Murrells Inlet, South Carolina.
We are a growing, independent, cash-pay functional medicine and wellness practice in Murrells Inlet, South Carolina, specializing in bioidentical hormone replacement therapy, hormone optimization, metabolic health, genetics, and personalized wellness care.
Our mission is simple:
To inspire, encourage, and lead people to a better destination in their health.
We believe healthcare should be proactive rather than reactive, personalized rather than standardized, and built around relationships rather than transactions.
Declare Wellness Center is looking for an experienced Nurse Practitioner or Physician Assistant who wants to practice medicine differently.
Here, you’ll have the time and support to build meaningful relationships with patients, understand what’s actually happening with their health, and help guide them toward measurable improvements in how they feel and function.
Why This Position Is Different
We believe there is a better way to practice medicine.
Our patients come to us because they want a proactive, personalized approach to their health. They are invested in understanding their bodies, improving their health, and actively participating in the process.
Instead of managing a high-volume schedule of disconnected visits, you’ll build long‑term relationships with patients and follow their progress through labs, hormone optimization, nutrition, lifestyle interventions, genetics, and other individualized therapies.
You’ll also work closely with our clinical founder, health coaches, and support team rather than being expected to figure everything out on your own.
For a provider who loves learning, connecting the dots, and helping people become healthier, this can be an incredibly rewarding way to practice.
Specialized Clinical Training Provided
Previous functional medicine or BHRT experience is welcomed, but it is not required.
We are willing to train the right experienced clinician who is curious, coachable, and genuinely interested in health optimization.
You’ll receive structured mentorship and hands‑on training in areas including:
Our goal isn’t simply to teach you protocols. We want to help you develop the clinical reasoning and confidence to understand why you’re making recommendations and how to individualize care for the person sitting in front of you.
For the right provider, this is an opportunity to develop a highly specialized clinical skill set while being paid to practice it.
As part of our clinical team, you’ll progressively develop your own patient relationships while working closely with our founder and clinical support team.
Your responsibilities will include:
