Job summary

The Phoenix at Opelika is seeking a Wellness Nurse to join their team. The LPN reports directly to the Wellness Director and helps monitor the health and wellbeing of residents while leading wellness initiatives across programs.

Responsibilities include medication administration, coordinating with families and support services, and maintaining clinical quality assurance. Weekend and evening availability is required to meet peak service delivery times.

Qualifications

• Licensed Practical Nurse (LPN) or Registered Nurse (RN) with current license.
• Minimum 1 year experience with elders or disabled individuals preferred.
• Demonstrated leadership capabilities in a care setting.
• Knowledge of Assisted Living and Dementia care preferred.
• Ability to work weekends, evenings and flexible hours.

Responsibilities

• Monitor health and wellbeing of residents and support wellness programs.
• Medication care administration and documentation.
• Communicate with resident families and support services.
• Support PSP and wellness visits; document changes in care needs.
• Assist Wellness Director with clinical sections of monthly wellness activities.
