Job summary

The Neighborhood at Grove Park in Alabama is seeking a Wellness Nurse/Nurse Med Tech to join their team. The LPN reports to the Wellness Director and works Monday through Friday with every other weekend required, from 7am to 7pm.

You will monitor residents' health, lead care activities, administer medications, and coordinate with families and support services. Prior experience with elders or disabled individuals and leadership ability are valued.

Qualifications

• Must be a Licensed Practical Nurse or Registered Nurse.
• Minimum of one year experience with elders or disabled individuals.
• Leadership capabilities required.
• Experience in Assisted Living or Dementia care preferred.
• Ability to work weekends, evenings and flexible hours.

Responsibilities

• Identifies ongoing needs and services of residents through the assessment / PSP process.
• Ensures proper follow through and documentation for changes in clinical care needs.
• Assist Wellness Director with completing monthly wellness visits and PSP to reflect the resident monthly.
• Informs the Wellness Director and/or Executive Director of any changes in medication or service level that may result in a higher tier level or potential move-out.
• Assist with obtaining weights and vital signs monthly for each resident prior to completing monthly wellness health updates.
• Assist with direct resident care as needed.
