The Phoenix at Opelika is seeking a dedicated LPN (Nurse Med Tech) to join our wellness team. The role reports to the Wellness Director and focuses on monitoring residents' health, medication care administration, and leading wellness initiatives.

Responsibilities include updating PSPs, documenting care changes, and communicating with families and support services. Weekend/evening shift flexibility is required to meet peak service times.

Qualifications

• Must hold an active LPN or RN license.
• Minimum one year of experience with elders or disabled individuals.
• Leadership capabilities and ability to work weekends/evenings.
• Experience in Assisted Living or Dementia care preferred.
• Flexible hours and weekend availability.

Responsabilités

• Identifies ongoing needs and services of residents through the assessment / PSP process.
• Ensures proper follow through and documentation for residents with a change in clinical care needs.
• Assist the Wellness Director with completing the clinical sections of monthly wellness visits and PSP to accurately reflect the resident monthly.
• Informs the Wellness Director and/or Executive Director of any changes in medication or service level that may result in a higher tier level or potential move-out.
• Assist with obtaining weights and vital signs monthly for each resident prior to completing monthly wellness health updates.
• Assist with direct resident care as needed.
