Job summary

The Neighborhood at Vestavia Hills, part of Phoenix Senior Living, is seeking a Wellness Nurse/Nurse Med Tech to join their team on the 7pm–7am shift. The LPN reports to the Wellness Director and provides leadership support and resident care.

Key duties include medication administration, engaging with resident families, maintaining clinical quality, and using PSP/Wellness processes. Must be a licensed LPN or RN with at least one year caring for elders, and available weekends/evenings.

Qualifications

• Must be a Licensed Practical Nurse or Registered Nurse.
• Minimum of one year of experience working with elders or disabled individuals.
• Must have demonstrated Leadership capabilities.
• Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred.
• Ability to work weekends, evenings and flexible hours, available to our customers at peak service delivery days and times.

Responsibilities

• Identifies on‑going needs and services of residents through the assessment / Personalized Service Plan (PSP) process.
• Ensures proper follow through and documentation for residents with a change in clinical care needs.
• Assist the Wellness Director with Completing the clinical sections of monthly wellness visits and PSP to accurately reflect the resident monthly.
• Informs the Wellness Director and/or Executive Director of any changes in medication or service level that may result in a higher tier level or potential move-out.
• Assist with obtaining weights and vital signs monthly for each resident prior to completing monthly wellness health updates.
• Assist with direct resident care as needed.

Education

LPN or RN license

1 year elder care experience
