Job summary

The Neighborhood at Paducah, a Phoenix Senior Living community, is seeking a compassionate LPN or RN to join our wellness team. The Wellness Nurse/Nurse Med Tech will oversee resident health, support care plans, and assist with medication administration and family communication.

Qualifications

• Must be a Licensed Practical Nurse or Registered Nurse.
• Minimum of one year of experience with elders or disabled individuals.
• Leadership capabilities preferred.
• Experience in Assisted Living or Dementia care preferred.
• Ability to work weekends, evenings, and flexible hours.

Responsibilities

• Monitor health and wellbeing of residents and support wellness initiatives.
• Provide medication care administration and document changes.
• Communicate with families and resident support services.
• Support Wellness Director and assist with monthly wellness visits and PSP updates.
• Lead, demonstrate and support wellness program components.
