Job summary

Qualifications

• RN licensed in New York State; BSN preferred.
• At least 1 year of clinical experience with severe mental illness preferred.

Responsibilities

• Develop individualized wellness and health action plans with clients.
• Conduct wellness checks and health-risk assessments.
• Provide health education including diabetes management and co-morbidity education.
• Coordinate care with housing staff, Health Homes, and treatment teams.
• Monitor medication regimens and provide clinical guidance.
• Document activities and records per agency policies.
• Participate in staff meetings and ongoing trainings.
