Job summary

South Shore Association dba SAIL is seeking a registered nurse to provide wellness education and wellness checks within Supportive Housing and Mobile Residential teams in Nassau County, NY. The role emphasizes prevention and collaboration with housing staff and clinicians.

The RN will develop health action plans, conduct assessments, coordinate post-discharge services, monitor medications, and document all activities in accordance with agency policies and funding requirements.

Qualifications

• Registered Nurse with NYS licensure; BSN preferred.
• One year of clinical experience with severe mental illness preferred.

• Strong organizational and communication skills for interaction with staff, clients, families, and the community.
• Demonstrates critical thinking and sound judgment.

Responsibilities

• Develop individualized wellness and health action plans with clients, coaching on weight, nutrition, stress management, and smoking cessation.
• Conduct wellness checks and health-risk assessments to identify risks like diabetes and hypertension.
• Deliver health education including diabetes self-management and co-morbidity education.
• Coordinate care with housing staff, Health Homes, and treatment teams; arrange post-discharge services.
• Monitor medication regimens and provide clinical guidance to the team.
• Document education, activities, interventions, and records per agency policies.
• Attend meetings and trainings as required and complete other duties.

• At least one (1) year clinical experience providing assessment and treatment to individuals suffering with severe mental illness preferred.

General Description

Responsible for providing wellness education and wellness checks to clients in the Supportive Housing, Mobile Residential Team with an emphasis on prevention.

Duties and Responsibilities

• Develop individualized support, wellness, and health action plans with clients, including coaching on weight management, nutrition, stress management, smoking cessation, and other meaningful health goals.
• Conduct wellness checks, health-risk assessments, biometric screenings, and other individualized programs to identify and address risks such as diabetes, high blood pressure, high cholesterol, and heart disease.
• Provide health education, including diabetes self-management and co-morbidity education, and monitor programs in accordance with expectations and funding requirements.
• Coordinate care with housing staff, Health Homes, treatment teams, and other providers; participate in hospital discharge planning and arrange post-discharge services, equipment, and in-home follow-up.
• Monitor medication regimens as requested and provide clinical expertise, guidance, and education to team members.
• Document education, activities, interventions, and all required records accurately and promptly in accordance with agency policies and procedures.
• Participate in staff meetings, supervision, agency meetings, committees, and relevant workshops or conferences; follow agency policies and complete other assigned duties.
