Job summary

S:US is seeking Wellness Coaches (Case Managers) to assist individuals in accessing medical, psychological, and social services by developing strong referral networks. This in-person role helps residents manage housing and recovery goals, linking them to community supports and coordinating services.

You will conduct group meetings, review service plans, and support residents in achieving rehabilitation and housing stability.

Qualifications

• Bachelor’s Degree in Human Services field required.
• Two years experience with individuals with mental illness, substance abuse issues and/or homelessness required.
• Experience in scattered site setting preferred.
• Experience in group work, tenant organization, case management, referral, and/or community outreach preferred.
• Bilingual English/Spanish preferred.
• Valid NYS driver’s license preferred.

WELLNESS COACHES (CASE MANAGERS)

As a Wellness Coach you will assist consumers with accessing needed community services (medical, psychological, substance abuse, vocational, food, clothing, etc.) through the development of a referral network.

This is great opportunity for those looking to build a career in the not-for-profit social services arena. With a passion to help others, you can easily progress into other positions such as Vocational Specialist, Clinical Coordinator, Program Director.

Responsibilities:

• Engage people with mental illness and assist them in meeting the demands of community living and maintaining their housing by teaching them the necessary skills, and linking them to services that will support their recovery, independence and self-determination
• Conduct Group Meetings on Wellness Management
• Complete service plan reviews for all residents
• Regularly assess, correct and report apartment and personal living space inspections conditions as assigned
• Provide Case Management for residents, engage residents in setting rehabilitation and housing support goals, and collaborate with them in developing a Housing Support Plan where indicated.

Qualifications

• Bachelor’s Degree in Human Services field required
• Two years experience with individuals with mental illness, substance abuse issues and/or a history of homelessness required
• Experience in scattered site setting strongly preferred
• Experience in group work, tenant organization, case management, referral, and/or community outreach strongly preferred
• Experience or training in peer counseling or advocacy preferred
• Working knowledge of various computer programs including e-mail, Microsoft Word, Excel, PowerPoint and Windows Operating System
• Bilingual English/Spanish preferred
• Valid NYS driver’s license preferred
