Job summary

S:US in New York is seeking a Wellness Coach to assist program participants with housing stabilization, access to services, and the development of a referral network. You will provide case management, develop service plans, accompany participants to appointments, advocate for clients, and coordinate with supportive services while documenting activities and participating in team supervision.

• Identify daily living skill problems and provide hands-on assistance or referral.
• Support facilities management to maintain apartment standards.
• Provide supportive counseling as needed.
• Conduct regular home and hospital visits as necessary.
• Document all services in compliance with agency standards.
• Collaborate with social service teams on activities and community inclusion.
• Liaison to community services and participate in supervision and trainings.
• On-call participation for crises and emergencies.
• Other duties as assigned.

Position Overview

SCOPE OF ROLE: The Wellness Coach will assist program participants with housing stabilization, support services and accessing needed community services (medical, psychological, substance abuse, vocational, food, clothing, etc.) through the development of a referral network, referral and follow up.

ESSENTIAL DUTIES & RESPONSIBILITIES:

• Provide case management to assigned caseload.
• Develop and monitor individual service plans in conjunction with agency program participants
• Assist program participants with problem solving and crisis intervention.
• Accompany program participants on medical, psychiatric, and public benefit appointments, asnecessary.
• Act as advocate for program participants as appropriate; encourage and support self-advocacy.
• Identification of any problems in daily living skills; hands on assistance and/or referral inaddressing problems in daily living skills.
• Support Facilities Management staff in ensuring maintenance of apartment standards asoutlined by S:US and other diverse funders.
• Supportive counseling to program participants as needed.
• Regular home visits and hospital visits, as necessary.
• Maintenance of a strong working relationship with all therapeutic support services to ensurethe best quality of care.
• Documentation of all services in compliance with agency standards.
• Work as a member of the social service team in the development of off-site recreation, socialization, educational and skill building activities with a view towards community inclusion and integration.
• Liaison to community-based services and groups including civic, social service government, andbusiness organizations.Participate in ongoing individual and group supervision, team meetings, case conferences,division meetings and trainings.
• Adhere to on call cell phone system to facilitate crisis intervention, emergencies and othersituations that require immediate action.
• Other duties as assigned.

Qualifications

REQUIREMENTS:

REQUIRED EDUCATION AND EXPERIENCE

• Bachelor’s degree in Social Work or related human services field, with two years of experience working with individuals with mental illness, substance use issues and/or a history of homelessness, or in the human services field.
• Experience in group work, tenant organization, case management, referral, and/or communityoutreach strongly preferred.
• Experience or training in peer counseling or advocacy preferred.
• Computer literate in Microsoft suite, skill sets in Word, Excel, and Outlook.

PREFERRED QUALIFICATIONS & SKILLS

• Bilingual English/Spanish speaking preferred.
