ChristianaCare's Weight Management Department in Wilmington, DE is seeking an experienced Case Manager - RN to deliver high-quality, safe, and evidence-based patient care. The Case Manager serves as the primary clinical resource for patients throughout their weight management journey, providing comprehensive care coordination, patient education, clinical triage, and navigation services.

Qualifications

• Experience in weight management, obesity medicine, bariatric services or related field.
• Proven care coordination and navigation experience.
• Strong documentation and communication skills.

Responsibilities

• Coordinate care across weight management services and team members.
• Provide clinical triage and guide patients through their care journey.
• Collaborate with physicians, dietitians, behavioral health specialists, pharmacists and community partners.
• Document accurately and ensure timely follow-up.
• Work in a hybrid environment with proficient use of healthcare IT systems (Epic).
