Job summary

ChristianaCare's Weight Management Department in Wilmington, DE seeks an experienced Case Manager - RN to deliver high-quality, coordinated care across the patient journey. You will provide education, clinical triage, and navigation services while collaborating with a multidisciplinary team in a hybrid work setting.

The ideal candidate has RN with BSN preferred and 2+ years medical-surgical experience, with bariatric experience preferred.

Qualifications

• RN with BSN preferred and 2+ years medical-surgical experience; bariatric experience preferred.
• Experience in care coordination, patient navigation, and population health is valued.
• Proficiency with Epic or similar healthcare IT systems required.

Responsibilities

• Coordinate weight management care and obesity/bariatric services across the patient journey.
• Provide clinical triage, assess patient needs, and guide through care pathways.
• Collaborate with physicians, dietitians,Behavioral health, pharmacists and partners; document timely follow-up.
• Build relationships with patients and the care team; manage priorities in a hybrid environment.
