Job summary

ChristianaCare CareVio is hiring an RN Case Manager to coordinate care for a complex patient population with medical and social determinants of health needs.

The role involves telephonic case management, collaboration with a multidisciplinary team, and development of care plans across the continuum. A DE RN license, BLS, and CCM or ACMA certification within 18 months are required, with opportunities for advanced certification.

Qualifications

• Registered Nurse with BSN required.
• Active DE RN Licensure or Compact Licensure in good standing.
• Ability to obtain a non-compact state RN Licensure as needed.
• Active Basic Life Support (BLS) certification.
• Case Management Certification required within 18 months of eligibility.
• Advanced Certification recommended.

Responsibilities

• Perform ongoing telephonic case management activities including assessment, planning, coordination, and evaluation.
• Collaborate with patients and care team to develop a member-centered plan of care.
• Develop, implement, evaluate, and revise case management care plans.
• Identify barriers to care and propose removal strategies.
• Promote member wellness and refer to community resources or disease management programs.
• Coordinate with providers and facilities to ensure timely and cost-effective care.
