Job summary

ChristianaCare's Weight Management Department in Wilmington, DE is seeking an experienced Case Manager - RN to deliver high-quality, safe, and evidence-based patient care. The Case Manager serves as the primary clinical resource for patients throughout their weight management journey, providing comprehensive care coordination, patient education, clinical triage, and navigation services.

Working collaboratively with physicians, advanced practice providers, dietitians, behavioral health

Qualifications

• Active RN license in Delaware.
• BSN preferred.
• 2+ years in weight management, obesity medicine, or bariatric services preferred.
• Strong communication, documentation, and teamwork skills.
• Bilingual English/Spanish preferred.

Responsibilities

• Coordinate care and navigate patients through weight management services.
• Provide patient education and clinical triage as needed.
• Collaborate with physicians, dietitians, behavioral health specialists, pharmacists, and community partners.
• Maintain accurate documentation and timely follow-up.
