Responsibilities

• Respond to changes in resident condition and coordinate interventions.
• Assist with admissions, discharges, transfers as needed.
• Communicate with families, physicians, and on-call leadership.
• Ensure accurate documentation per facility policies.
• Promote a positive team environment and weekend leadership coverage.
• Act as on-site clinical leader during weekend shifts.
