Qualifications

Knowledge of electronic medical record systems is required.

Responsibilities

Communicates patient and staff information to interdisciplinary teams and care managers daily.
Consults with Triage Nurse or Nursing Manager regarding risk before Crisis Care or GIP status.
Completes documentation timely and accurately for patient contact activities.
