Job summary

Mederi Caretenders Home Health, part of LHC Group and a member of the Optum family, is hiring a Registered Nurse to provide in-home clinical care on weekends. This full-time role coordinates care with physicians and the interdisciplinary team, conducts OASIS assessments and medication reconciliation, supervises aides, and supports patients and families in their homes.

Qualified candidates hold an active RN license in the state, a valid driver’s license, and CPR certification; weekend schedule

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Valid driver’s license with access to a dependable vehicle or public transportation.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Develops and updates patient plans of care in collaboration with physicians and the multidisciplinary team; ensures home‑health eligibility and completes comprehensive nursing assessments (including OASIS and medication reconciliation).
• Provides and coordinates high‑quality clinical care; supervises home health aides and LPN/LVNs in compliance with regulations and required supervisory visits.
• Documents all patient care per policy and payer requirements; communicates with physicians, patients, families, and staff for coordinated care.
