Job summary

Optum is seeking a Full Time, Salaried Home Health Weekend Registered Nurse (RN) to provide and coordinate home-based nursing care on a Friday/Saturday/Sunday OR Saturday/Sunday/Monday schedule. Responsibilities include creating and updating care plans, supervising aides, maintaining documentation, and ensuring safe, quality patient outcomes in home settings.

Required: current RN licensure and a valid driver’s license with reliable transportation.

Qualifications

Current and unrestricted RN licensure in state of practice.
Current Driver's License with access to a dependable vehicle or public transportation.
Preferred: CPR Certification (within 90 days).
1+ years of clinical experience (preferred).

Responsibilities

Develops, implements, and updates patient plans of care with physicians and the interdisciplinary team; ensures home-health eligibility and completes comprehensive nursing assessments (OASIS and medication reconciliation).
Provides and coordinates high-quality clinical care; supervises home health aides and LPN/LVNs in compliance with regulations; conducts supervisory visits.
Documents all patient care per policy; communicates with physicians, patients, families, and staff to ensure coordinated care.
Participates in orientation and precepting of new staff; contributes to performance improvement and patient safety initiatives.
Demonstrates professionalism, punctuality, ongoing learning, and participates in on-call/weekend rotations.
Ability to function in diverse home settings regardless of client demographics.
