Job summary

Mederi Caretenders Home Health, part of the LHC Group and aligned with Optum, is seeking an RN for home health with PRN status and potential for full-time. You will deliver nursing care in patients' homes as prescribed and coordinate with physicians and the care team to maintain continuity of care.

The role requires current RN licensure in the state, a valid driver's license, and reliable transportation. CPR certification is preferred, with strong communication and independent work skills

Qualifications

• Current and unrestricted RN licensure in state of practice.
• CPR certification preferred or within 90 days of hire.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed nurses, provides instruction, and assigns tasks according to regulations.
• Provides supervisory visits and ensures compliance with plan of care and policy.
• Performs OASIS assessments, medication reconciliation, and comprehensive patient documentation.
• Communicates with physicians, patients, families, and team members to ensure quality care and continuity.
