Job summary
Optum, through Baptist Trinity Home Care, is seeking a Full-Time Weekend Registered Nurse to provide and coordinate home health care for patients in their homes, Friday/Saturday/Sunday or Saturday/Sunday/Monday schedules available. Qualifications
- Active RN licensure in state of practice.
- Valid driver's license and access to reliable transportation.
- CPR certification or ability to complete within 90 days of hire.
Responsibilities
- Develops and updates patient plans of care with physicians and the team, ensuring eligibility and comprehensive nursing assessments.
- Provide and coordinate high-quality clinical care; supervise home health aides and LPN/LVN staff per regulations.
- Document all patient care accurately and communicate with physicians, patients, families, and staff.
- Participate in orientation, contribute to performance improvement and patient safety initiatives.
- Demonstrate professionalism, punctuality, and adherence to agency standards; participate in on-call/weekend rotations.
- Ability to work with diverse patients in various home settings.
