Job summary

Elite Home Health, part of LHC Group and within the Optum family, seeks a Weekend Registered Nurse to deliver and coordinate in-home nursing care as prescribed by the physician and compliant with laws and agency policy.

You will develop and implement the plan of care, supervise aides, and collaborate with physicians, families, and the care team to ensure continuity of patient care and safety. Texas RN license and reliable transportation are required.

Qualifications

• Current, unrestricted RN license in the state of practice (Texas).
• Valid driver’s license and access to dependable transportation or vehicle.

Responsibilities

• Initiates and revises the plan of care in collaboration with the physician and health care team.
• Supervises home health aides and licensed nurses; provides instruction and assigns tasks per regulations.
• Provides supervisory visits and ensures quality, timely documentation and compliance.
