Job summary

Specialized Home Care is seeking a compassionate Registered Nurse (RN) Case Manager for weekend coverage in Wichita, Kansas. You will work one-on-one with patients at home, managing and coordinating all aspects of their care with strong clinical skills and independent work style.

Responsibilities include conducting patient assessments, collaborating with families and physicians, documenting care plans, and participating in case conferences.

Qualifications

• Minimum 1 year of nursing experience; 2+ years preferred in acute or post-acute settings.
• Active State Registered Nurse licensure is required.
• Proof of current CPR certification and valid Driver's license.

Responsibilities

• Oversee patient care from admission through discharge by executing the Plan of Care.
• Collaborate with patient, family/caregiver(s), physicians, and care team members.
• Submit timely and complete clinical documentation and progress notes.
• Participate in case conferences to review services and patient goals.
• Assist with on-call rotation as assigned.
