Job summary

Wesley Medical Center in Wichita, KS is seeking an RN Case Manager PRN to join the Case Management Team. This role covers inpatient and observation patients, coordinating discharge planning and care transitions with physicians, families, and external agencies.

The ideal candidate holds Kansas RN licensure, BLS certification, and 3 years of hospital nursing experience, with preferred case management or critical care experience. PRN, no guaranteed hours, with competitive benefits.

Qualifications

• Current licensure in Kansas as a Registered Nurse (RN)
• Current Basic Life Support (BLS) through AHA/Red Cross - Healthcare Provider CPR
• 3 years of experience in a clinical hospital setting
• Bachelor's degree from an accredited institution is preferred
• 2 years of case management or 2 years of critical care experience preferred
• Certification in case management or utilization review preferred
• InterQual experience preferred

Responsibilities

• Provide case management services for inpatient and observation patients
• Identify patients at risk for adverse outcomes during transitions between care settings
• Perform comprehensive psychosocial, medical, and discharge needs assessments
• Evaluate progression of care using InterQual criteria
• Document recommendations, discharge plans, care coordination, and case management activities
• Serve as a liaison among physicians, patients, families, hospital staff, and outside agencies
• Seek ways to control costs without compromising safety, quality, or services delivered
• Advocate for patient rights, needs, and under-resourced populations
