Job summary

Phoenix Home Care and Hospice is hiring a Home Health RN Case Manager in Wichita, KS. The role is full-time, Monday-Friday, with a pay-per-visit model and a base of $78,000-$88,000. You’ll manage a caseload, supervise LPN visits, and coordinate therapy teams.

New RN hires receive 4-6 weeks of paid training, a ride-along option to see the role, and PTO plus floating holidays accruing from day one. Mileage is reimbursed on each visit.

Qualifications

• RN license for Kansas or Compact required.
• Strong organizational and clinical skills.
• Excellent communication with patients and team members.

Responsibilities

• Manage a caseload of home health patients and supervise LPN visits.
• Coordinate with Physical Therapy, Occupational Therapy, and Speech Therapy teams; build care plans.
