Job summary

Phoenix Home Care & Hospice in Wichita, KS is seeking an RN Case Manager to provide compassionate nursing care to clients with terminal diagnoses and collaborate with the interdisciplinary team to develop patient-centered plans.

Responsibilities include educating patients and families, coordinating with physicians, performing comprehensive assessments, and guiding care decisions to improve quality of life. This full-time role follows a Monday–Friday days schedule, with pay range $75,000–$90,000.

Qualifications

• Kansas or Compact RN license required.
• Valid driver's license and auto liability insurance required.

Responsibilities

• Educate patients and families about care plans and decisions.
• Consult with the attending physician and Hospice Medical Director on changes to care plans.
• Coordinate hospice care with the care team to meet patient needs.
• Perform initial and ongoing comprehensive assessments.
• Conduct informational visits and hospice care evaluations.
