Qualifications

• Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired.
• High level of clinical knowledge in home health care is highly preferred.
• Experience with high acuity patients is a plus.
• Self-Directed and organized.
• Experience working with an interdisciplinary team is highly desired.

Responsibilities

• Conduct Start of Care (SOC) Admissions - OASIS.
• Make home visits to patients in designated territory.
• Monitor clients’ conditions and report changes to the clinical team.
• Initiate and administer IV medication safely and effectively.
• Accurately document observations and evaluations pertaining to patient care.
