Job summary

A home health care agency in the Oakland Area is seeking a Part-time Registered Nurse Case Manager. The role involves conducting Start of Care admissions and home visits to patients in the designated territory, ensuring high-quality patient care. Candidates must hold a current CA RN license and have experience with assessments and iv medication infusions. Qualifications

• Valid CA driver’s license.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.

Responsibilities

• Conduct Start of Care (SOC) admissions – OASIS.
• Make home visits to patients in designated territory.
• Collaborate with physicians and agency staff to deliver the best care possible.
