Independence Health Systems is seeking a Weekend RN Case Manager to coordinate patient care and discharge planning across the care continuum. The role requires assessment, planning, and collaboration with the health team to optimize clinical and financial outcomes.

The RN will ensure safe transitions, maintain documentation, and support patients and families throughout the care journey in collaboration with the interdisciplinary team.

Qualifications

• Three years clinical experience in healthcare.
• Recent case management experience.
• Proficient documentation skills.

Responsibilities

• Assess, plan, coordinate, monitor and evaluate care for patients and families to promote effective care coordination.
• Manage transitions of care to reduce readmissions and improve outcomes.
• Coordinate discharge planning and arrange referrals and community resources as needed.
• Collaborate with interdisciplinary teams to ensure seamless patient care and communication.
• Document care plans and maintain confidential patient records.
• Support patient and family education and self-management for ongoing care.
