Qualifications

• Associates degree from an accredited RN program and Ohio RN license.
• Minimum 3–5 years recent acute care clinical experience.
• Part-time weekend coverage availability.

Responsibilities

• Identify discharge needs and coordinate level of care transitions to prevent readmission.
• Provide clinical information to outside agencies.
• Communicate with care team effectively and professionally.
