Job summary

Carilion Clinic is seeking an RN Case Manager for inpatient care at the Roanoke facility. The role focuses on case management, discharge planning, and coordination with the care team to optimize patient outcomes and resource use.

The position requires Virginia RN licensure, a Bachelor’s degree, and strong communication skills. Certification in utilization review and MDM collaboration may be beneficial. This is a full-time, on-site opportunity with standard daytime hours.

Qualifications

• Registered Nurse with Virginia license.
• Bachelor's degree required; 5 years RN hospital experience may substitute.
• AHA BLS-HCP within 6 months of hire.
• Strong communication and problem-solving skills.

Responsibilities

• Provide case management for assigned patient populations.
• Coordinate care and services across the health care continuum.
• Collaborate with Utilization Review Nurse and Social Work.
• Ensure documentation reflects patient status and plan of care.
• Facilitate referrals to home health, hospice, and DME.
• Promote efficient discharge planning and care transitions.
