Lifecare Home Health Family is seeking an experienced Clinical Care Manager to serve as the clinical point of contact for a home care caseload. This role blends nursing judgment with care coordination, acting as triage for medical concerns and caregiver issues, often in a single call.

The ideal candidate is an RN with strong triage, communication, and problem-solving skills, comfortable working remotely with occasional in-home visits as needed.

Qualificações

• Active RN license in Florida.
• 2–3 years of clinical nursing experience in home health, hospice, or care management preferred.
• Experience with EVV or care management software is a plus.

Responsabilidades

• Triage clinical calls and assess urgency; guide actions.
• Coordinate care plans with providers, caregivers, and families.
• Monitor patient conditions and escalate to physicians when needed.
• Document assessments and care plan changes per policy and regulations.
