• Active RN license in good standing.
• Prior experience in home health, intake, referral management, case coordination, or clinical triage preferred.
• Strong understanding of home health services, documentation, and care coordination processes.

Aufgaben

• Provides oversight of patient care services and personnel, including assignments, referrals, and care plan updates.
• Serve as first line for resolution of staff concerns and issues.
• Plans and participates in delivery of home health services, coordinating with providers and facilities.
• Coordinate with hospitals, physicians, discharge planners, visiting clinicians, patients, and internal teams.
• Provide training, education, and performance reviews for staff.
• Communicate with leadership on continuous quality improvement and problem solving.
• Collaborate with Nursing Director, Intake, QA, and branch staff to support efficient workflow.
• Support Lombard and Chicago branch operations as needed.
