Job summary

Lifecare Home Health Family in Florida is seeking a Clinical Care Manager to serve as the clinical point of contact for home care clients, families, and caregivers. This RN role blends nursing judgment with hands‑on care coordination, acting as the first line of triage for medical concerns, caregiver performance issues, scheduling conflicts, and interpersonal challenges that arise in the home care setting.

Schedule: Weekends and Holidays.

Qualifications

• Active RN license in the state of practice.
• 2–3 years of clinical nursing experience; home health, hospice, or care management preferred.
• Working knowledge of geriatric and chronic conditions (hypertension, UTIs, AMS, skin issues).
• Experience with EVV or care management software (e.g., AlayaCare) preferred.
• Strong triage and clinical decision‑making; know when to escalate.
• Ability to de‑escalate emotionally charged situations with patients, families, and staff.

Responsibilities

• Serve as first clinical contact; triage symptoms and determine urgency and response.
• Coordinate with providers, caregivers, and external agencies regarding care plans and changes.
• Oversee medication management and reconciliation per regulations and scope.
• Support caregiver performance issues, scheduling conflicts, and transitions in care.
