Responsibilities

• Assess chronic disease management needs and psychosocial risk factors.
• Collaborate with PAS, financial counselor and UM nurse to review insurance requirements.
• Develop individualized discharge plans with patient/family and physicians.
• Participate in interdisciplinary rounds to confirm discharge date and level of care.
• Document interactions and care progression in the EMR.
• Identify and resolve barriers to discharge and optimize resource use.
