Job summary

Qualifications

Responsibilities

• Collaborate with neonate, adolescent, adult, and geriatric patients, families and staff to determine appropriate care settings and discharge planning.
• Assess post-hospital care needs and coordinate related services with community agencies; provide support for social drivers of health.
