Job summary

Qualifications

• Graduate of an accredited Registered Nursing School (RN).
• Current License in the State of Georgia as a Registered Nurse or Nursing Compact (NLC/eNLC) or Multistate License.
• Two (2) years previous clinical experience.

Responsibilities

• Collaborate with clinicians to determine the appropriate setting of care and hospitalization.
• Assess post-hospital care needs and coordinate discharge planning.
• Provide support to address social drivers of health.
