Job summary

Northside Hospital is seeking a Registered Nurse to evaluate patient needs across neonate to geriatric populations, coordinate appropriate care settings, and assist with discharge planning and post-hospital support.

The RN will work with families, physicians and hospital staff to ensure optimal care and smooth transitions, with emphasis on effective communication and patient advocacy. The role requires strong interpersonal skills and accurate documentation.

Qualifications

• Graduate of an accredited Registered Nursing School (RN)
• Current Georgia RN license or Nursing Compact (NLC/eNLC) or Multistate License
• Two (2) years prior clinical experience.
• Preferred BSN
• Three (3) years of experience in case management, social services or related field.
• Certified Case Manager.
• Excellent oral and written communication skills.
• Ability to type accurately at approximately 30 wpm.

Responsibilities

• Collaborate with patients, families, medical staff and community agencies to determine appropriate care setting and hospitalization needs.
• Assess post-hospital care requirements and coordinate discharge planning.
• Provide support to address social drivers of health.
