Job summary

Amedisys is seeking a Weekend Admit RN Home Health to join our team for Friday–Sunday shifts (or other weekend options). You will perform start-of-care assessments, complete OASIS, and provide skilled nursing care in patients’ homes, coordinating with families and care teams.

Required: an active RN license in relevant states, at least 1 year of RN experience, CPR, and a valid driver’s license with reliable transportation. Competitive pay and comprehensive benefits are offered.

• Minimum 1 year RN clinical experience; VP approval if less.
• Current CPR certification.

Responsibilities

• Performs start-of-care comprehensive assessment and develops plan of care.
• Completes OASIS assessments timely and accurately.
• Provides skilled nursing care in accordance with plan of care and policies.
• Coordinates care with staff, physicians, family, and resources.
• Initiates referrals when indicated.
• Promotes patient health through teaching and rehab measures.
• Assists patients in self-care learning.

Responsibilities

• Performs patient start of care comprehensive assessment, evaluation, development and implementation of plan of care.
• Completes OASIS assessments timely and accurately.
• Provides skilled nursing care to patients in accordance with the plan of care and agency policies.
• Helps to achieve and maintain continuity of patient care by communicating information with individual staff, physicians, family members and community resources.
• Initiates appropriate referrals when indicated.
• Promotes and maintains patient health through teaching and appropriate rehabilitative measures.
• Assists patients in learning appropriate self-care techniques.
• Other duties as assigned.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• One year of clinical experience as an RN. If less than one year clinical experience, you must be approved by VP of clinical.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
