Job summary

Amedisys is seeking an admissions registered nurse in Crestview Hills, KY. You will lead patient start of care, perform OASIS assessments, and deliver skilled nursing care with a focus on one-on-one patient relationships.

Qualifications

• RN license valid in the state you will serve.
• Minimum 1 year of clinical RN experience; approval by VP of clinical if less.
• Current CPR certification is required.
• Valid driver’s license with reliable transportation.

Responsibilities

• Perform start of care assessment and develop the plan of care.
• Complete timely and accurate OASIS assessments.
• Provide skilled nursing care per plan and policies.
• Coordinate information with staff, physicians, families, and resources.
• Initiate referrals when indicated to support patient care.
• Promote patient health through teaching and rehabilitative measures.
• Teach patients self-care techniques and ensure continuity of care.

• Performs patient start of care comprehensive assessment, evaluation, development and implementation of plan of care.
• Completes OASIS assessments timely and accurately.
• Provides skilled nursing care to patients in accordance with the plan of care and agency policies.
• Helps to achieve and maintain continuity of patient care by communicating information with individual staff, physicians, family members and community resources.
• Initiates appropriate referrals when indicated.
• Promotes and maintains patient health through teaching and appropriate rehabilitative measures.
• Assists patients in learning appropriate self-care techniques.
• Other duties as assigned.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• One year of clinical experience as a n RN. If less than one year clinical experience, you must be approved by VP of clinical.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
