Job summary

UnitedHealth Group in Sylva, NC is seeking a Registered Nurse in Home Health to provide and direct nursing care to patients in their homes as prescribed by physicians, coordinating care and ensuring continuity of care.

You will supervise aides and licensed nurses, perform supervisory visits, and deliver clinical services with strong infection control and documentation. Requires NC RN licensure and 1+ year home health experience.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• 1+ years of Home Health experience preferred.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• Current CPR Certification or ability to complete within 90 days of hire preferred

Responsibilities

• Initiate, develop, implement, and revise the plan of care with physician collaboration.
• Supervise home health aides and licensed nurses, providing instruction and assigning tasks.
• Provide supervisory visits.
• Deliver high-quality clinical services within scope and infection control standards.
• Complete comprehensive OASIS assessments and medication reconciliation accurately.
• Document patient visits per policy and payer requirements.
• Make initial/comprehensive nursing evaluation visits for home health eligibility.
• Communicate information timely with agency staff.
• Participate in orientation of new staff and serve as preceptor.
• Participate in on-call and weekend rotations as needed.

About This Role

As the Registered Nurse in Home Health, you will provide and direct nursing care to patients in their homes as prescribed by the physician, ensuring compliance with applicable laws and agency policies. This role is ideal for nurses who are dedicated to coordinating care and maintaining continuity for patients.

What You'll Do

• Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other health care professionals.
• Supervise care provided by home health aides and licensed practical/vocational nurses, providing instruction and assigning tasks.
• Provide required supervisory visits.
• Deliver high-quality clinical services within the scope of practice and infection control standards.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Document patient visits per policy and payer requirements, syncing timely per LHC policy.
• Make initial and/or comprehensive nursing evaluation visits to ensure patients meet home health eligibility.
• Communicate relevant information timely and effectively with appropriate agency staff.
• Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students.
• Participate in on-call and weekend rotation as needed to meet patient needs.

Requirements
