Concord Post Acute in Concord, CA is seeking a compassionate Physical Therapist Assistant (PTA) to join our in-house rehabilitation team on a PRN/per-diem basis. The role focuses on delivering skilled treatments and collaborating with other care providers to support residents’ recovery journeys.

The starting pay is $50–$53 per hour (DOE). A current California PTA license in good standing is required, and the position offers flexible scheduling within a supportive, team-based environment.

Qualifications

Active California PTA license in good standing.
PRN/per-diem availability.
Collaborate with nursing staff and therapists on resident care.

Responsibilities

Provide skilled physical therapy treatments under the direction of the supervising Physical Therapist.
Accurately document daily treatments, progress notes, and patient outcomes.
Collaborate with nursing staff, therapists, and interdisciplinary team members regarding resident care plans.
Participate in patient care conferences and ongoing treatment planning.
Assist in training and supporting Restorative Aides to promote continuity of care.
Educate patients, families, and caregivers on maintenance programs and safe discharge planning.
Maintain professional, compassionate communication with residents, families, and staff.
