Develops individualized plans of care with patients and designated care partners, aligned with cultural beliefs and values, and participates in ongoing professional growth within a collaborative healthcare team.

Qualifications

• Specialized Diploma Graduate of an accredited school of nursing.
• Patient-and Family-Centered Practice knowledge of how to convey respect and collaborate with each patient, family and/or designated care partner in care and care planning.
• Cultural Awareness knowledge of people with diverse backgrounds and the ability to convey respect and connect effectively.
• Effective Communication understanding of communication concepts and techniques to convey ideas, information, and needs with patients and families.
• Patient Assessment and Care knowledge of assessing ailments and injuries and implementing a care plan.
• Discharge Planning knowledge of post-discharge needs and collaboration with patient and family.
• Medication Administration and Prescription knowledge of medications and post-discharge instructions.
• Evidence-Based Practice and ethical decision making in nursing care.

Responsibilities

• Interacts with co-workers, patients, their family/designated care partner(s), and other staff reflecting Jefferson's values.
• Partners with patients and families to identify learning needs and provide clear education.
• Uses information technology to communicate, manage knowledge, mitigate error, and monitor outcomes.
• Works within inter-professional teams to ensure safe, compassionate, coordinated care.
• Creates an environment supporting learning, growth, and preceptorship for staff.
• Participates in hospital committees and seeks feedback for practice improvement.
