Job summary

Qualifications

• Specialized Diploma graduate of an accredited school of nursing required.
• Patient- and Family-Centered Practice: collaborate with each patient, family and/or designated care partner in care and care planning.
• Cultural Awareness: knowledge of people with diverse backgrounds and ability to convey respect.
• Effective Communication: transmit, receive, and interpret ideas and information with patients and teams.
• Patient Assessment and Care: assess ailments and implement a treatment plan.
• Patient and Designated Care Partner Education: elicit learning priorities and educate patients and partners.
• Medication Administration and Prescription: knowledge of medications and administration.
• Discharge Planning: partner with patient and family in discharge planning and follow-up.
• Assessment and Diagnosis: use tools for patient assessment and nursing diagnosis.
• Health Information Documentation: document histories, vital signs, medications, diagnoses, plans and results.
• BLS - Basic Life Support (AHA)

Responsibilities

• Integrates evidence-based practice with patient and family preferences for delivery of quality care.
• Partners with patients and families in identifying learning needs and provides education.
• Minimizes risk by following nursing practices and protocols and communicates with the team.
• Uses information technology to communicate, manage knowledge, and support decision making.
• Monitors outcomes and participates in quality improvement to enhance care safety.
• Collaborates with inter-professional teams and participates in safety and quality committees.
