You will develop personalized care plans, collaborate with the care partner(s), and participate in safety and quality initiatives while mentoring staff and supporting evidence-based practice.

Qualifications

• Specialized Diploma graduate required.
• Active RN license in state of practice required.
• BSN strongly preferred within 3 years of hire.
• Excellent communication and cultural awareness for diverse patients.
• Knowledge of health information documentation and clinical protocols.

Responsibilities

• Interacts with patients and families to provide education and support.
• Integrates evidence-based practice with clinical expertise and patient preferences.
• Collaborates with the care team to ensure safety and high-quality care.
• Uses information technology to communicate, manage knowledge, and mitigate error.
• Participates in committees and quality initiatives to improve safety.
• Assists in discharge planning and follow-up arrangements.
• Actes as a primary preceptor to appropriate staff members.
