The Gallatin Center for Rehabilitation and Healing is seeking a Physical Therapist for a PRN weekend position to support our long-term care residents. The successful candidate will assess referred patients, provide direct care, and develop individualized treatment plans.

You will supervise Physical Therapy Assistants, document progress, and communicate with physicians, patients, and families. Experience in geriatric care and strong documentation skills are preferred.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Licensed/registered PT in state of practice.
• Experience supervising Physical Therapy Assistants.
• Geriatric/long-term care experience preferred; not required.
• Strong documentation and reporting skills.
• Ability to evaluate, plan, implement and discharge patients.

Responsabilités

• Assess referred patients and provide direct patient care.
• Develop and administer individualized treatment plans with goals.
• Determine patient capacity and tolerance for rehabilitation.
• Document patient condition and progress; educate patients and families.
• Supervise Physical Therapy Assistants and coordinate care.
• Collaborate with all disciplines for patient-centered care.
• Maintain HIPAA confidentiality and professional standards.
• Adhere to schedules and demonstrate professionalism.
• Perform evaluations, progress notes, recertifications and discharge documentation.
• Report to Director of Rehabilitation.
