First Day Homecare - Michigan is seeking an LPN for 1:1 skilled nursing care in Burt, MI. The role involves performing nursing functions under medical orders and state regulations, with supervision by a medical practitioner.

The schedule is Friday to Monday, day shifts 8:00 a.m. – 6:00 p.m., with a focus on safe, patient-centered care and collaboration with the care team.

Qualifications

• Must have current LPN license to practice in the state.
• CPR certification required.
• Demonstrated written, verbal, and interpersonal communication skills.

Responsibilities

• Provide professional nursing care as defined by state Nurse Practice Act.
• Follow physician orders and client care plans.
• Document changes in orders in the medical record per agency procedures.
• Perform clinical duties including medication administration and client teaching.
• Collaborate with the care team to ensure coordinated care.
• Maintain safety for clients and coworkers.
• Adhere to agency policies and accreditation standards.
• Participate in care coordination and agency meetings.
• Perform other duties as needed.

• Adult aged client in Burt, MI. Day shift nurse needed Friday-Monday 8a-6p

Job Summary

Performs skilled nursing functions in accordance with federal, state, and local laws and within the guidelines of professional standards and the Nurse Practice Act. All nursing services provided will be under the direction of medical practitioner (MD, DO, NP, PA, etc.) orders.

Responsibilities

• Provides professional nursing care as defined in the state Nurse Practice Act.
• Provides care in accordance with medical practitioner orders and the client’s care plan.
• Maintains orders for clients and documents all changes in orders in the medical record following Agency procedures.
• The scope of clinical practice includes personal care services, monitoring and reporting findings, medication administration, client teaching, emergency intervention, and specialized pediatric services such as tracheostomy care, ventilator management, and tube feedings.
• Works collaboratively with other members of the client’s care team.
• Promotes personal safety and a safe environment for clients and coworkers.
• Performs job in compliance with Agency policies, procedures, and applicable payor and accreditation standards.
• Participates in the coordination of care and attends Agency meetings as requested.
• Performs other related duties and responsibilities as deemed necessary.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current license to practice as an LPN in the state of operation.
• Current CPR certification.
• Demonstrated written, verbal, and interpersonal communication skills.
• Licensed driver with automobile insured in accordance with state and/or Agency requirements.
