Infinite Medical P.C. in Fall River, MA, is seeking a Primary Care NP/PA to provide on-site care in long-term care settings. This part-time role covers weekend hours and emphasizes meaningful patient relationships within a collaborative care environment.

You will assess, diagnose, and manage chronic and acute conditions, order tests, and coordinate with facility staff to optimize resident health and well-being.

Qualifications

• Active and unrestricted NP/PA license in good standing.
• Massachusetts license in good standing.
• Minimum 6 months of long-term care or skilled nursing facility experience.
• Strong communication, clinical and critical thinking skills.
• Passion for elder care and person-centered healthcare.

Responsibilities

• Provide primary care to residents in skilled nursing and long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and goals.
• Maintain a safe, respectful environment for residents.
