Infinite Medical P.C. in Fall River, MA is hiring a Part-Time Primary Care NP/PA to provide on-site care in long-term care facilities, primarily on weekends.

This role emphasizes meaningful patient relationships and collaborative care with facility teams. You will perform exams, manage chronic and acute conditions, order tests, develop individualized care plans, and educate residents and families about treatment options.

Qualifications

• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
• State-specific license in good standing in Massachusetts (MA).
• Minimum 6 months of experience in long-term care or skilled nursing facilities.
• Strong communication, clinical, and critical thinking skills.
• Passion for elder care and person-centered healthcare.

Responsibilities

• Provide primary care services to residents in long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
