Infinite Medical P.C. is seeking a Primary Care NP/PA to provide on-site care for residents in long-term care facilities. This part-time weekend position emphasizes meaningful patient relationships and collaborative care.

Active MA licensure is required, with experience in elder care preferred. Competitive compensation, benefits, and malpractice coverage are included as part of the package.

Qualifications

• Active and unrestricted Nurse Practitioner or Physician Assistant license.
• State MA license in good standing.
• Minimum 6 months of experience in long-term care or skilled nursing facilities.

Responsabilités

• Provide primary care services to residents in skilled nursing and long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and labs.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and care goals.
• Ensure a safe, respectful environment for residents.
